中風後才偵測到的心房顫動 (Atrial fibrillation detected after stroke, AFDAS)

加拿大西安大略大學 (Western University)的Dr. Luciano Sposato,在2017年一篇文章時首先提出”AFDAS”(Atrial fibrillation detected after stroke)的觀念[1]。AFDAS定義是在急性中風或短暫缺血性症狀發作(transient ischemic attack)後才發現病患有心房顫動,發現的時機點可能是在急診,住院中或是出院後在門診追蹤時才發現。但回顧Dr. Sposato的文章,會發現陸續於2012年開始,Dr. Sposato即針對此一議題提出相當深入的觀察。2012年時,Dr. Sposato發現,若是在中風後病患接受持續性心臟節律追蹤,比起未持續追蹤心律的病患,有較高的比例發現陣發性心房顫動(18.2% vs. 2.2%, p=0.005),而平均發現病患有心房顫動的時間為2天內[2]。在2013年,Dr. Sposato分析275位急性中風病患,其中23位(8.3%)有新診斷的心房顫動,而這些病患比起原本已知心房顫動的病患,有較少的左心房擴大(left atrial enlargement)及較高比例的腦島中風(insular cortex stroke); 而比起中風後正常心律的患者,這些病患有較大的腦中風面積及較高比例的腦島中風。因此Dr. Sposato推測這些中風後才偵測到的心房顫動也許有較高的機會是因為中風本身所導致(neurogenic mechanism)[3]。

 

2015年,發表在Lancet Neurology的meta-analysis,Dr. Sposato分析50篇研究, 共11658位病患,共23.7%的患者在中風後才偵測到心房顫動[4],因此約莫有1/4左右的中風病患在中風後可能被偵測到有心房顫動。同年度發表的meta-analysis更進一步發現,高比例的中風後心房顫動時間不超過30秒[5],與一般常見的持續性的心房顫動似有不同。因此,Dr. Sposato正式於2017年提出AFDAS的名詞,並進一步將AFDAS的成因分成神經性(neurogenic type)及心因性(cardiogenic type) 或是混合兩者 (mixed type)。Neurogenic AFDAS主要是因為大腦調節中樞(cerebral regulation center,包含insular cortex,或是有些學者認為的cingulate gyrus或是pre-frontal cortex)受損, 進而造成短暫及較低風險的AFDAS。若是病患心臟結構本身有問題(例如左心房擴大或是心衰竭等),則病患原本中風前可能就有心房顫動,只是在此次中風後才被注意到,此類患者則被歸類到cardiogenic AFDAS,促成中風的風險可能較neurogenic AFDAS高。根據上述的理論基礎,Dr. Sposato認為兩者所導致的AFDAS可能對於中風的風險有所不同,並因此進一步於2018年的文章進行驗證。在2018年,Dr. Sposato分析過去23376位病患資料,將病患分為正常心律(normal sinus rhythm, SR),中風後才偵測到的心房顫動(AFDAS),及已知的心房顫動(known atrial fibrillation, KAF)[6]。AFDAS病患族群為10.1%,KAF病患族群為29.5%。KAF患者比起AFDAS及SR族群確實有較高的比例具有心衰竭,心肌梗塞,及冠狀動脈疾病等心臟相關問題;而AFDAS及SR族群兩族群則無統計上差異。Dr. Sposato進一步分析此三類患者一年後的再度中風風險。經過不同模式的風險校正,發現KAF族群的一年再中風風險高於SR族群,而AFDAS族群的一年再中風風險則與SR族群無統計上之差異。因此佐證了Dr. Sposato理論。目前由Dr. Sposato主持正在進行中的PARADISE (Pathophysiology And Risk Of Atrial Fibrillation Detected After Ischemic Stroke) study[7] 則涵蓋了動物實驗,臨床及流行病學研究等三個面向,進一步深入探討此一議題,也許在未來,我們可以更了解AFDAS的成因及對於中風所導致的風險,也更可以了解此類病患的中風治療應當如何進行。
